Job Description


Assertive Community Treatment Team Housing Specialist- Full‑Time 

Location: Newark, MD
Salary Range: $40,000–$55,000 annually
Schedule: 4‑day, 36-hour work week — Monday through Thursday

Position Summary

This position will be responsible for planning and implement housing services for clients who have co-occurring mental illness and substance abuse disorders. This position is fully dedicated to CCBHC Expansion Grant.

Key Responsibilities

  • Maintain strict confidentiality in all client interactions and documentation, ensuring full compliance with HIPAA, CARF standards, and Maryland state regulations.
  • Participate in agency-required trainings and professional development activities as directed by the supervisor.
  • Attend weekly agency housing meetings and relevant community housing forums; build and maintain collaborative relationships with local shelters and housing providers.
  • Contribute to the development and implementation of person-centered treatment plans, including ongoing monitoring and accurate documentation of client progress.
  • Provide case management and supportive counseling services, assisting clients with symptom management and housing stability.
  • Adhere to ACT evidence-based practice standards by delivering high-quality, recovery-oriented services and maintaining a minimum of 15 hours of direct client contact per week.
  • Coordinate care with clients’ natural supports and collaborate with community service providers to promote treatment engagement and stabilization.
  • Perform additional duties as assigned by the supervisor to support team and program operations.


Qualifications 

  • High School Diploma or equivalent
  • Current CPR and First Aid certification.
  • Valid full driver's license with a safe driving record.
  • Excellent verbal and written communication skills.
  • Experience working with vulnerable populations.
