Student Success Navigator





Reports to: Director of Student Health & Wellness


 


Qualifications: 



  • Bachelor’s degree in School Work; Counseling; Psychology; Public Health; or related field

  • Knowledge of adolescent development, trauma-informed care, and substance use issues.

  • Strong interpersonal and communication skills with the ability to engage resistant youth.

  • Ability to collaborate with diverse stakeholders and maintain confidentiality.


Preferred Qualifications:



  • Master’s degree in counseling, social work, or related field.

  • Licensed or certified in counseling, social work, or substance abuse (LCSW, LCAS, LPC, CADC, etc.).

  • Experience in case management, youth mentoring, or substance use prevention/intervention.


Major Responsibilities and Duties:



  • Conduct outreach to identify and engage youth at risk for or struggling with substance use.

  • Build trusting, supportive relationships with youth and their families.

  • Provide screenings and assessments to determine needs and readiness for services.

  • Connect youth with appropriate treatment, counseling, or community resources.

  • Assist families in navigating complex systems, including schools, behavioral health, juvenile justice, and social services.

  • Offer motivational interviewing, mentoring, and coaching to encourage healthy choices.

  • Collaborate with school staff, healthcare providers, and community partners to ensure a coordinated approach.

  • Maintain accurate documentation and case records in compliance with program and funding requirements.

  • Monitor progress and provide consistent follow-up to ensure engagement in services.

  • Educate youth and families about substance use, risk factors, and protective strategies.

  • Performs related duties as assigned by the Director of Student Health and Wellness 


 


Working Conditions:   


Must be able to use a variety of office equipment such as computers, scanners, and copiers.  Must be able to communicate effectively.   Must be able to work in an office environment and come into direct contact with school system staff, students, external agencies, and the community.  At times requires the ability to lift, carry, push, pull or otherwise move objects up to ten pounds.  The work frequently requires driving automotive equipment.  Due to the amount of time spent standing and/or walking, physical requirements are consistent with those for sedentary work.
