Job Title: Clinical Liaison – RN or LPN | Skilled Nursing Facilities
Location: Antigo, WI & Laona, WI (Wausau Market)

Champion Care is seeking a passionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Clinical Liaison for our skilled nursing facilities in Antigo and Laona, Wisconsin. This is a key role focused on driving census growth, managing hospital relationships, and coordinating smooth transitions of care for both facilities.
Key Responsibilities:
Serve as the primary liaison between local hospitals and Eastview Health and Rehabilitation Center and Nu Roc Health and Rehabilitation Center.
Review clinical information and determine eligibility for admission to our facilities.
Develop and execute strategic marketing plans to increase visibility and drive census.
Build and maintain strong relationships with discharge planners, case managers, and hospital staff.
Collaborate with internal clinical and admissions teams to ensure timely and appropriate admissions.
Monitor market trends and identify new referral sources.
Represent Champion Care at networking and community events.
Qualifications:
Active RN or LPN license in the state of Wisconsin (required)
Experience in skilled nursing, healthcare marketing, case management, or admissions preferred
Strong clinical assessment and decision-making skills
Excellent communication and relationship-building abilities
Ability to travel regularly between Antigo and Laona
