We are currently seeking a part-time Social Worker to join our interdisciplinary team and provide meaningful care and emotional support to patients and their loved ones during end-of-life care.
This is a field position assigned to the Community East Team, with a primary work assignment that spans the Greenwood Lake, Warwick, Pine Island and Florida region.
The work schedule for this position will be Monday, Wednesday, and Friday, 9:00 am - 3:00 pm, totaling 18 hours per week.
This position is non-exempt and is not eligible to participate in our benefit plans.
The Social Worker must possess a deep understanding of the hospice philosophy and the ability to effectively educate patients and families about available services. Some key responsibilities include conducting in-home visits, completing comprehensive psychosocial assessments, delivering clinical counseling and case management support, collaborating with the other members of the interdisciplinary team to develop and implement individualized care plans, and offering bereavement counseling and emotional support to our patients and their families.
Requirements
Education and Experience:
- A graduate of a master’s program in social work accredited by the Council on Social Work Education.
- Valid NYS Licensed Master Social Work/Licensed Clinical Social Work.
