Key Responsibilities:
• Conduct psychosocial assessments for new cancer diagnoses and develop intervention plans with appropriate referrals.
• Document patient needs, interventions, and discharge planning in compliance with hospital policies.
• Provide supportive counseling, grief education, and facilitate patient/family access to community resources.
• Communicate and collaborate with physicians, interdisciplinary team members, and community agencies to support patient care.
• Act as a patient navigator by coordinating care, appointments, education, and resources to prevent delays in treatment or discharge.
• Lead support groups, provide community education, and serve as a patient advocate throughout the continuum of care.
Qualifications:
1. Associate's or Bachelor's Degree in a healthcare-related field (e.g., Nursing, Allied Health, Social Work, or Public Health) required.
2. Clinical experience in healthcare or social services settings is preferred.
3. Strong communication, assessment, and care coordination skills; knowledge of healthcare systems and resources.
