Location: Salt Lake City Area | Schedule: Typically, Monday - Friday 8:00 am – 5:00 pm | Type: Full-Time | FLSA: Non-Exempt

What You'll Do

As a Housing Case Manager, you'll work closely with clients to provide holistic housing case management and connect them with vital resources that support long-term success. This role is about more than services-it's about building relationships, removing barriers, and helping families move forward with confidence.

Key responsibilities include:

  • Partnering with clients to assess social and financial barriers and develop personalized action plans.
  • Providing ongoing support and follow-up to ensure clients successfully access services and referrals.
  • Connecting families with community resources such as healthcare, mental health services, financial literacy programs, job training, education, childcare, and transportation.
  • Maintaining detailed and timely case notes in the agency's electronic data system.
  • Collaborating with community partners and representing the agency in a professional and positive way.
  • Empowering clients to understand the connection between their circumstances and choices, helping them build sustainable life skills.
  • Continuously evaluating the effectiveness of referrals to ensure clients receive the most appropriate and timely support.

Required Qualifications

  • Bachelor's degree in Social Work or a related field
  • Prior case management experience
  • Proficiency in Microsoft Office
  • Valid driver's license

Preferred Qualifications

  • Bilingual (English/Spanish)
  • Experience working with low-income populations
  • Background in conflict resolution or trauma-informed care

Additional Information

This role may involve local travel, including the use of a personal or agency vehicle. Most work is seated with occasional walking, standing, or light lifting (up to 10 lbs).
