Job Title: RN Case Manager – Hospice Care Beyond the Hospital (1:1 Patient Focus)
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area, 21-County Service Area (territories assigned based on coverage needs)

What You’ll Do
• Manage a hospice caseload with full ownership of patient care and coordination.
• Deliver pain and symptom management focused on comfort and dignity.
• Serve as the primary RN for patients and families, guiding them through end-of-life care.
• Collaborate with physicians, social workers, chaplains, aides, and the interdisciplinary team.
• Educate families and caregivers with clarity, compassion, and confidence.
• Provide care in homes, assisted living, and skilled nursing settings.
• Maintain accurate, timely EMR documentation.

What You’ll Bring
• Active Minnesota Registered Nurse license.
• Experience in hospice, oncology, ICU, or palliative care.
• Strong clinical judgment and confidence working independently.
• Passion for patient-centered, relationship-driven care.
• Excellent communication and critical thinking skills.
• Reliable transportation and ability to travel throughout the service area.
