Registered Nurse Job Description – Long Term Care Facility

Oversight of Social Worker and Treatment Nurse Duties

Position Title: Registered Nurse (RN) – Long Term Care Facility

Key Responsibilities:

  • Carry out social worker duties, including resident assessments, care planning, psychosocial support, and coordination of services.

  • Ensure all social work documentation, interventions, and communications meet CMS and HHSC standards.

  • Oversee treatment nurse duties for the remainder of the day, including wound care, medication administration, and monitoring of resident health conditions.

  • Provide guidance and supervision to treatment nurses, ensuring compliance with regulatory requirements and facility policies.

  • Collaborate with interdisciplinary teams to promote resident well-being and address care needs.

  • Maintain accurate records, reports, and documentation as required by CMS and HHSC.

  • Participate in staff meetings, trainings, and quality improvement initiatives.

  • Will be in the on-call rotation with the DON and ADON every 3 weeks.

Qualifications:

  • Active RN license in the state of employment.

  • Experience in long term care, social work, or treatment nursing preferred.

  • Knowledge of CMS and HHSC guidelines and regulations.

  • Strong communication, leadership, and organizational skills.

  • Ability to manage multiple responsibilities and prioritize tasks effectively.

Schedule: Partial day dedicated to social worker duties; remainder of day overseeing treatment nurse responsibilities.
