Oversight of Social Worker and Treatment Nurse Duties
Position Title: Registered Nurse (RN) – Long Term Care Facility
Key Responsibilities:
Carry out social worker duties, including resident assessments, care planning, psychosocial support, and coordination of services.
Ensure all social work documentation, interventions, and communications meet CMS and HHSC standards.
Oversee treatment nurse duties for the remainder of the day, including wound care, medication administration, and monitoring of resident health conditions.
Provide guidance and supervision to treatment nurses, ensuring compliance with regulatory requirements and facility policies.
Collaborate with interdisciplinary teams to promote resident well-being and address care needs.
Maintain accurate records, reports, and documentation as required by CMS and HHSC.
Participate in staff meetings, trainings, and quality improvement initiatives.
Will be in the on-call rotation with the DON and ADON every 3 weeks.
Qualifications:
Active RN license in the state of employment.
Experience in long term care, social work, or treatment nursing preferred.
Knowledge of CMS and HHSC guidelines and regulations.
Strong communication, leadership, and organizational skills.
Ability to manage multiple responsibilities and prioritize tasks effectively.
Schedule: Partial day dedicated to social worker duties; remainder of day overseeing treatment nurse responsibilities.
