Job Description

Job Title: Elite Wound Care Nurse Practitioner (AGNP/CNP) | Senior Living Rounding.

Company: Twin Cities Physicians.

Compensation: $105,000 to $120,000 annually + high-impact clinical autonomy.

Benefits: Full Benefits Package (FT status only).

Location: Greater Twin Cities Metro Area, MN.

Schedule: Full-Time or Part-Time | Flexible, Self-Directed Territory.

Position Summary:
This is not your traditional Nurse Practitioner role. As a Wound Care focused NP with Twin Cities Physicians, you will operate with a high level of autonomy, trust, and clinical influence, managing complex wounds while building meaningful, long-term relationships with your patients. You will be the go-to expert within your assigned communities, delivering advanced care where it matters most. If you are energized by ownership of your practice, variety in your day, and the ability to directly impact outcomes, this opportunity was built for you. Wound care certification or strong experience is highly preferred and will set you apart.

Responsibilities:
• Provide on-site primary and wound care services across senior living communities.
• Lead and manage advanced wound treatment plans, improving healing outcomes and quality of life.
• Drive proactive care management, including chronic conditions and preventative interventions.
• Collaborate closely with physicians, nursing staff, and interdisciplinary teams to ensure seamless care delivery.
• Communicate consistently with families and community partners, building trust and transparency.
• Order and interpret labs, manage medications, and ensure accurate clinical documentation.

Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota.
• Experience in geriatrics, primary care, or wound care strongly preferred.
• Valid driver’s license and reliable transportation.
• A genuine passion for serving the senior population with empathy and excellence.
• Strong communication skills with the ability to lead clinical conversations confidently.
