What You�ll Do

  • Lead and complete MDS assessments and care plans in compliance with federal and Medicare guidelines

  • Coordinate Medicare A (PPS) scheduling and maintain accurate timelines

  • Ensure documentation is accurate, timely, and audit-ready

  • Collaborate with nursing, therapy, and interdisciplinary teams

  • Identify and respond to significant changes in resident conditions

  • Guide and support staff on RAI processes and documentation best practices

  • Oversee and maintain the Restorative Nursing Program

  • Play a key role in driving quality care and reimbursement accuracy

What We�re Looking For

  • Active RN license in Indiana (required)

  • Long-Term Care experience (required)

  • Strong attention to detail and organizational skills

  • Clinical confidence and critical thinking ability

  • Team-oriented mindset with strong communication skills
