Job Description

Position Title: Registered Nurse - Healthcare for the Homeless

Location: Carolyn Downs Family Medical Clinic in Seattle and various locations in Seattle Area, WA (On-site)

Employment Type: 24 hours per week

Union: Yes

 

Salary Range: $41.22-61.54 per hour, Non-Exempt, payable bi-weekly. The salary scale for this position is between steps 0 and 19, depending on experience. This position is grant-funded, and its continuation is contingent upon the ongoing availability of funding.

About Us

Seattle Roots Community Health is a not-for-profit organization that operates two primary care clinics, an After-Hours Clinic, two School-Based Health Centers, a low barrier Medication-assisted treatment clinic and a Dental Clinic. Our Carolyn Downs Family Medical Center was founded in 1970 in Seattle's Central District. Our Country Doctor Community Clinic was established in 1971 by residents of the Capitol Hill neighborhood.

Job Summary

The Healthcare for the Homeless program is hiring a mission-driven outreach nurse to cover the Tiny House Villages and Shelter sites. This nurse will continue our work in promoting health equity.

 

Job Responsibilities

  • Treats patients in accordance with the mission and philosophy of Seattle Roots Community Health
  • Treats clients with respect and dignity. Display sensitivity to clients enduring homelessness, mental health issues or substance use disorder
  • Establish supportive therapeutic relationships with clients
  • Maintain confidentiality according to Seattle Roots policy. Obtain signed consent when possible
  • Comfortable working with patients experiencing substance use disorders, mental health conditions and homelessness
  • Embracing of harm reduction, motivational interviewing, and trauma-informed philosophies of patient-centered care
  • Strong interpersonal skills and effective problem-solving skills
  • Function in stressful situations and prioritize effectively with numerous competing priorities
  • Effective conflict resolution
  • Excellent verbal, non-verbal, and written communication skills, interpersonal skills, computer skills
  • Completes documentation in a timely manner

Provides Nursing Interventions

  • Triage clients with acute medical issues, provides health education, problem-solving, treatment (as needed), referral and follow-up care as needed
  • Facilitates care for medically complex or chronically ill clients as needed
  • Providing blood pressure checks and wound care/dressing changes as indicated
  • Assists clients with chronic disease management
  • Provide accurate and appropriate health education as needed
  • Provides referrals to community resources as needed
  • Assist clients in establishing a medical and dental home
  • Assists client with referrals for mental health and substance abuse issues
  • Parenting education and support (as needed)
  • Provides health education to site staff (as needed)
  • Collaborate with community partners
  • Participating in and complete necessary continuing nursing education

 

Qualifications

  • You hold a current RN license from WA state
  • You have experience providing care to medically underserved populations
  • You interact and work well with people from diverse cultures and backgrounds
  • You’re comfortable working with Microsoft Office, Google products, and on personal computers
  • Preferably, you have at least two (2) years of clinic nursing and triage experience and have case management experience
  • Preferably, you bring experience with Epic electronic medical records (EMR) software
  • Preferably, you can read, write, and speak Spanish to provide interpretation for our patients
  • This position is based at our Country Doctor Community Clinic; however, the selected candidate will be expected to travel between care sites. Reliable transportation is required.
