Implements family support services for emotionally disturbed children and adolescents. Provides support, information, referral, follow-up, and technical assistance to families and general consultation to staff on children and youth. Develops and maintains data collection system indicating the number of referrals, resources/services, unmet needs of families, and barriers to obtaining services for all interested parties.
If Family Partner is a QMHP-CS, the following duties are also required.
· Assist in accessing emotional, behavioral, social, educational, medical, and other appropriate services
using the Uniform Assessment tool.
· Assesses client’s needs and authorizes services to meet the identified needs.
· Coordinates the development of an Individualized Treatment Plan with measurable outcomes.
· Coordinates service delivery.
· Provide Crisis Prevention and Case Management services when needed.
· Monitors service delivery.
