We are seeking a part-time LPN to work every other weekend.
Position Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. This role is responsible for coordinating and overseeing resident health care services on a 24/7 basis in collaboration with the Director of Wellness. The LPN will provide clinical care, support resident service planning, supervise care staff, and ensure high-quality care for all residents.
Key Responsibilities:
Resident Care & Services:
- Provide direct nursing care, including medication administration, treatments, and health monitoring
- Conduct resident assessments, develop and update individualized service plans
- Communicate with physicians, families, and designated representatives regarding resident status and changes
- Respond to health emergencies and provide appropriate care within scope of practice
- Advocate for residents and ensure their physical, emotional, and social needs are met
- Maintain compliance with all healthcare regulations (HIPAA, OSHA, state regulations, etc.)
- Ensure high levels of resident satisfaction and promptly address concerns or complaints
Coordination & Oversight:
- Supervise and support care staff to ensure quality service delivery
- Coordinate services with outside healthcare providers and ancillary services
- Assist with resident admissions, discharges, and transitions of care
- Ensure accurate documentation, recordkeeping, and physician order transcription
- Oversee medication ordering and treatment plans
Staff Training & Leadership:
- Train and mentor care staff in proper procedures and resident care
- Identify training needs and support ongoing staff development
- Promote a culture that supports dignity, independence, and quality of life for residents
Administrative Duties:
- Maintain accurate resident records and reports
- Ensure compliance with policies, procedures, and infection control standards
- Participate in on-call rotation and assist with scheduling needs
Qualifications:
- Must be 18 years of age or older
- Active Licensed Practical Nurse (LPN) license in the state of employment
- Graduate of an accredited nursing program
- Minimum of one (1) year experience in home health, assisted living, or geriatric nursing preferred
- Strong communication and leadership skills
- Ability to work flexible hours, including weekends, holidays, and on-call shifts
- Ability to lift up to 50 lbs (30 lbs regularly)
- Current driver’s license, reliable transportation, and auto insurance
- First Aid certification (or ability to obtain within 90 days)
