Small SNF in Westchester County is seeking an MSW to serve as Director of Social Services for this 88-bed facility with both short- and long-term care residents.  Health Care experience required, nursing home experience preferred.  Position reports to the Administrator and is responsible for the facility case load.  Working knowledge of discharge planning, care planning, MOLST and Sigma Care system essential.  Salary and benefits to be discussed upon interview.    Please send resume to begin the process.
