Job Title: Hospice Scheduler LVN/LPN

Job Overview:


Scheduling Coordination:

  • Develop and maintain daily and weekly schedules for hospice staff, including nurses, social workers, and volunteers.
  • Coordinate shifts and on-call assignments to ensure coverage based on patient needs.

Communication:

  • Serve as the primary point of contact for hospice staff regarding scheduling inquiries and updates.
  • Communicate effectively with patients, families, and healthcare providers about scheduled visits and any changes.

Data Management:

  • Maintain accurate and up-to-date records in the scheduling database.
  • Ensure compliance with regulatory guidelines related to scheduling and patient care.

Problem-Solving:

  • Address scheduling conflicts proactively and implement solutions to ensure uninterrupted patient care.
  • Respond to urgent scheduling requests and adjust schedules as necessary.

Reporting:

  • Prepare and present scheduling reports to management, highlighting trends and areas for improvement.
  • Assist in tracking staff attendance and performance related to scheduling.

Requirements

   

Experience:

Previous experience in a healthcare setting, preferably in hospice or home health care.

High school diploma or equivalent; LVN/LPN preferred.


Skills:

Excellent organizational and time-management skills.

Strong communication and interpersonal skills.

Proficiency in scheduling software and electronic health records (EHR) systems.

Ability to work independently and as part of a team.

Compassionate and empathetic demeanor towards patients and families.
