    JOB SUMMARY





    •  





      • The Community Health Specialist supports the Southern Berkshire Rural Health Network in advancing health equity by developing, coordinating, and implementing system level strategies that address the social determinants of health. This role works collaboratively with community partners, healthcare providers, and local organizations to identify barriers and improve access to essential resources, with a primary focus on transportation, housing stability, and food security. The Specialist helps build sustainable community partnerships, strengthens cross sector communication, and supports initiatives that reduce disparities and improve overall well being for rural populations. Through data informed planning, outreach, and program support, the Community Health Specialist plays a key role in shaping equitable, community driven solutions that enhance health outcomes









  • JOB REQUIREMENTS






    • Experience:






      • 1 year of experience in community health, human services, public health policy and/or post graduate internship.  







    • Education:






      • Bachelor’s degree required;  Bachelor's degree in public health, public policy, social work, community development, communications or related field preferred. 







    • Licensure & Certifications:






      • None.









  • SKILLS & ABILITIES





    • Strong understanding of social determinants of health (SDOH’s), including housing stability, food insecurity, and transportation access.



    • Ability to build, facilitate, and sustain effective partnerships and coalitions across sectors.



    • Excellent written and verbal communication skills, with the ability to communicate complex topics and engage with diverse audiences.



    • Ability to manage multiple projects following work plans and work effectively both independently and collaboratively.



    • Experience living in or working with rural communities strongly preferred
