The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Licensed Practical Nurse – 24 Hour Coverage – Advanced Medical Services who demonstrates this mission and wants to work for an organization that makes a difference in the community.
What you will be doing?
The Licensed Practical Nurse (LPN) is responsible for the provision of nursing services to meet the identified health care needs of a designated population of clients under the direction of a Registered Nurse Trainer. Responsibilities include but are not limited to medication administration, enteral feeding and care, tracheostomy suctioning and care including respiratory treatments, indwelling catheter management and care, injections and other treatments under the delegation of a Registered Nurse Trainer. The LPN collaborates with physicians, other health care providers, and the Interdisciplinary Team, as needed, to address client advanced medical needs. The LPN works within the framework of AID’s policies and procedures to assure that accreditation standards and regulatory requirements pertaining to advanced medical services are met in all program locations assigned. The LPN is expected to participate in providing personal care to individuals as needed, including but not limited to dressing, toileting, bathing, etc. The LPN will assure client rights, dignity, and privacy are maintained while providing services; they participate in maintaining a clean and safe environment for the individuals supported. The LPN may work with clients with maladaptive behaviors, which can include but are not limited to verbal aggression, physical aggression, self-injurious behavior, property destruction, and elopement. The LPN is assigned to a primary location, however, must be able to support and/or accompany individuals while accessing the community including attending their healthcare provider appointments as required.
Essential Job Responsibilities
- Maintain a positive approach with solutions-focused language in working with clients, staff, and outside agencies.
- Implements Physician’s Orders, collaborating with physicians and pharmacists as needed.
- Completes clinical treatments and tasks including but not limited to enteral feeding and care, tracheostomy suctioning and care including respiratory treatments, indwelling catheter management and care, injections and other treatments under the delegation of a Registered Nurse Trainer
- Provides medication administration, monitoring, and/or training for clients as scheduled.
- Enters required documentation of services provided, assessments, and other clinical documentation as required to meet program and licensure requirements in a timely manner.
- Must have knowledge of client’s medical conditions and be able to monitor and relay information to and from medical appointments.
- Be familiar with and follow all agency and program rules and regulations. Reports concerns or issues to immediate supervisor without delay.
- Attend to the self-care needs of the clients including but not limited to toileting assistance, changing soiled garments, bathing and grooming, shaving, cleaning glasses and adaptive equipment, etc.
- Participates in the Person-Centered Planning process to assist in planning appropriate advanced medical services as well as community health services for clients, based on their assessed needs.
- Participate in the planning and implementation of community and in-house activities to ensure that advanced medical services are considered
- Maintains client confidentiality in accordance with agency policy and federal and state laws.
- Assure safe conditions inside and outside the home including maintenance and housekeeping. This includes but is not limited to reporting environmental safety concerns, participating in assigned safety drills and checklists monthly, assisting clients to participate in the upkeep of their homes, etc.
- Maintain a clean, sanitary environment, inside and out of all assigned buildings and vehicles, on a daily basis. This includes but is not limited to surface, deep, and sanitizing cleaning tasks as assigned. Recognize and respond to emergency situations that may occur and take appropriate action according to agency policies and procedures including completing required incident reporting or documentation.
- Travels between program and community locations to support clients’ participation in Community Day Services as well as accessing community health services.
- Attend and participate in regular staff meetings at program locations using solutions-focused language and positive approaches.
- Assist in the orientation/training of new staff using solutions-focused language and positive approaches.
- Demonstrate the ability to understand and follow directions (verbal and written) to successfully fulfill job responsibilities with minimal supervision.
- Must maintain LPN licensure in good standing by providing quality nursing services, completing continuing education requirements, and submitting timely licensure renewal paperwork.
- Must remain flexible to work scheduled hours at different locations based on client and program needs; location-specific training is provided.
- Must remain current in all agency training requirements to meet licensing and program requirements; includes attending live events as scheduled and completing online training modules prior to the due date.
- Excellent attendance and punctuality as defined by the program guidelines.
