Location: 6666 Manlius Center Road, East Syracuse, NY 13057

Pay Rate: $20.07 Hourly

Schedule: M-F 8:30a-4:30p

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.

About the Role

We are looking for a proactive employee to enhance the independence of and facilitate the care of persons with disabilities.

Responsibilities include:

  • Make home and collateral visits to individuals and their families to provide information, support and assist with acquisition of appropriate services

  • Provide information, advocacy and referral services by consulting with individuals, families, professionals and programs as well as doing any related research

  • Assist families and individuals through the eligibility process through OPWDD

  • Independently and accurately monitor budgets

  • Research community resources and provide linkage to increase inclusion within the participant�s community

Education & Experience

High school diploma/GED plus 3 years� experience working with individuals with disabilities, preferred. Associate�s degree in Human Service field plus 2 years� experience working with individuals with disabilities, preferred.

Knowledge, Skills & Abilities

Basic word processing and e-mail skills, required. Experience working with individuals with disabilities, preferred.

Conditions of Employment

Must possess a valid driver�s license and transportation necessary to meet program needs.
