Job Description

TITLE: RN Case Manager/Infection Preventionist

 REPORTS TO: Director of Nursing

Job Summary: The RN Case Manager is a registered nurse, licensed by the state, and has experience in geriatric nursing, rehabilitation, safety, critical thinking skills and team management.  Understands the importance of being a good leader and team player.  Demonstrates understanding and realizing the importance of each staff person in the CCC team. This position is responsible for developing and implementing an ongoing infection prevention and control program to prevent, recognize and control the onset and spread of infections to provide a safe, sanitary and comfortable environment. 

Qualifications: Must be registered and licensed with the State of Minnesota as a RN and must maintain licensure. must be generally familiar with regulatory readiness. The RN/IP will need to obtain the Infection Preventionist certification. The RN Case Manager is also required to have an ability to read, write and speak English and complete and pass a state background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

  • Follows the Clarkfield Care Center nursing service objectives and philosophy established by the Director of Nursing.
  • Follows standards of nursing practice. Leads by example with all staff.
  • Follows CCC nursing policy and procedures.
  • Schedules daily rounds to see and assess residents.
  • Ensure that all nursing services provided to the residents under his/her case management receive adequate and timely care, which is pertinent to the resident’s health status/condition.
  • Will be responsible to meet the resident/family needs by doing resident assessment, planning, evaluation and supervision of cares.
  • Assures the accuracy of resident records.
  • Participates in the evaluation of staff performance on her/his wing. Recognizes staff strengths and works toward improving their weaknesses.
  • Assures that a comprehensive plan of care is established and implemented for each resident and that the plan is reviewed at least quarterly and within seven days of the revision of the comprehensive resident assessment.
  • Follows the RAI process and consistently uses the MDS user’s manual for guidance on interpretive guidelines. Seeks out help when warnings come up or needing another opinion to correctly complete MDS assessments and Care Planning. Careful to follow MDS and RCC schedules.
  • Interacts with physicians to plan care of residents.
  • Contributes to identify quality of care, resident rights, quality of life, or facility practice problems.
  • Assures that nursing personnel caring for residents under her/his case management are following the resident’s care plan. Does teaching of staff of correct procedures.
  • Aware of good customer service and demonstrates the use of it.
  • Uses quality management skills and leadership skills to fulfill the mission of CCC.
  • Participates in department head meetings and communicates all pertinent information regarding resident care to other departments as each resident’s health status/condition changes.
  • Communicates and assists the Director of Nursing in any/all education / re-education of nursing staff persons.
  • Develops, implements and maintains an infection prevention program with input from DON and/or Administrator.
  • Coordinate and be actively accountable for the facility’s infection prevention and control program.
  • Establish facility-wide systems for the prevention, identification, reporting, investigation and control of infections and communicable diseases of residents, staff, and visitors.
  • Assist in developing and implement written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control.
  • Oversight of the facility’s antibiotic stewardship program.
  • Review and/or revise the facility’s infection prevention and control program, its standards, policies and procedures annually and as needed for changes to the facility assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
  • Follows appropriate safety and hygiene measures at all times to protect residents and themselves.
  • Acquires resources and means that promote a quality work environment; ensures quality performance and conformance with regulation and laws.
  • Assists and participates in the QA/QI process.
  • Keeps on top of and implements new programs or best practices.
  • Exercises sound judgment and decision making.
  • Responds appropriately to emergency needs in the facility (choking, falls, convulsions, fire and disaster situations, etc.)
  • Function as a nurse independently; this includes Floor Nursing RN duties, speaking with families, clarifying orders, passing medications and direct care as needed.
  • Demonstrates safe use of oxygen, lifts and all to her equipment; utilizes good body mechanics during all transfers and physical tasks.
  • Will participate on committees as necessary.
  • Other duties as directed.
