Job Summary:
The Physical Therapist provides skilled therapy services to patients in their homes, helping them regain independence, improve mobility, and achieve their goals in a setting that matters most to them.

Responsibilities include:

  • Evaluating and developing individualized treatment plans
  • Providing therapeutic interventions to improve strength, mobility, and function
  • Educating patients and families on care plans and progress
  • Collaborating with an interdisciplinary team to support patient outcomes
  • Completing timely and accurate documentation

Qualifications:

  • Current Physical Therapist license in Missouri
  • Home Health experience preferred, but not required
  • Strong clinical judgment and communication skills
  • A commitment to patient-centered care
