We are seeking a full-time licensed and motivated Physical Therapist Assistant (PTA) to join our in-house rehabilitation department full-time. This is a great opportunity to be part of a supportive, resident-focused care team committed to high-quality therapy and positive outcomes.

Key Responsibilities:

• Assist the Physical Therapist in implementing individualized treatment plans

• Provide direct care including exercises, gait training, and therapeutic activities

• Monitor and document residents’ progress and communicate updates to the PT

• Collaborate with interdisciplinary team members to support care goals

• Educate residents and caregivers on safe mobility practices and home exercise programs

• Complete accurate and timely documentation using electronic medical records

Qualifications:

• Current Vermont PTA license (in good standing – required)

• Previous experience in SNF or long-term care setting preferred

• Knowledge of rehab techniques and best practices

• Strong communication and team collaboration skills

• Familiarity with EMR systems preferred
