Impact Health is looking for a Palliative Care Nurse Practitioner 



On a PER Diem Basis (approx. 1-2 days per week)



to cover our Marlborough MA, Territory!!

Compensation and Benefits:




  • Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)

Key Responsibilities




  • Deliver person-centered care using evidence-based practices

  • Collaborate with interdisciplinary facility teams

  • Conduct assessments, develop treatment plans, and manage follow-up care

  • Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)

  • Educate patients, families, and staff

  • Complete timely documentation (within 24 hours)

  • Compliance with facility, state and federal regulations/protocols.
