This employee will work as part of the PCMH team with the following essential duties and responsibilities:



The RN is part of CFH multi-disciplinary team that provides clinical support to medical and behavioral health practitioners, in accordance with Article 28 and Patient-Centered Medical Home (PCMH). The clinical support provided is based on patient care needs and scope of the practice and follows the principles of Trauma-Informed Care. Provision of care is carried out utilizing nursing processes within the nursing scope as established by the New York State Board of Nursing, American Nursing Association scope and standards of practice, and other regulatory standards, in addition to CFH Policy and Procedures.



The RN will fulfill patient care responsibilities as assigned which may include and are not limited to: reviewing patient resource schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, and dressing, administering medication, treatment and/or vaccines. All job functions are carried out in support of the CFH’s mission and vision statements.



RN ESSENTIAL DUTIES AND RESPONSIBILITIES:




  • Under the supervision of the Director of Nursing, the RN is responsible for completing patient intake/screening, providing prepared patient education, and discharging patients.

  • Provide Patient Education 

  • Retrieve laboratory and radiology reports for the medical or behavioral health practitioners to review.

  • Obtain and accurately document the patient’s family and social history, vital signs (i.e. blood pressure, pulse, temperature, and pain), height, weight, oxygen saturation, and body mass index (BMI). Performs basic audio and visual screening.

  • Administer vaccines, medications, or treatments and fulfill specific orders as indicated by the Practitioner (i.e. intramuscular, etc.).

  • Perform point of care testing (i.e. urinalysis, rapid strep, rapid flu test, INR, phlebotomy, urine pregnancy test, and glucose testing).

  • Remove sutures or staples under the supervision of a physician or other appropriately licensed person, provide that the patient is evaluated, at a minimum, immediately before the procedure by an appropriately licensed professional.

  • Assist Practitioner with examinations and procedures.

  • Prepare and organize exam room supplies. Prepare the exam table, ensure supplies are not expired and are maintained in an organized manner in the cabinets, ensure all necessary equipment and supplies are readily available for the Practitioner's use.

  • Maintain emergency bags, oxygen tanks, refrigerator logs, and other clinical equipment per CFH protocols.

  • Maintain supply inventory, order medications, immunizations, and other supplies. Maintain accurate count and records of controlled substances.

  • Clean, disinfect and pack instruments for sterilization, maintain instrument sterilization logs.

  • Perform duties in accordance with Department of Health (DOH) guidelines and practice accepted infection control measures in accordance with OSHA blood-borne pathogens standards, National Patient Safety Goals, and the organization's policies and procedures.

  • Perform clerical functions, including but not limited to: patient registration, appointment check-in/check-out, appointment scheduling, verifying insurance, updating PCP, applying sliding fee as appropriate, answering the telephone, relaying messages, assisting Practitioner with completing medical forms, and scan documents into the patient's EMR.
