Job Description

Description

Assists the Case Management staff by performing clerical duties related to case management and discharge planning. Has full access to patient health information.

Assists with coordination and completion of forms including but not limited to:

  • FL-2 forms - CM staff review and doctor’s signature for extended care facility placements
  • Important Message from Medicare - obtaining patient/family signatures prior to discharge
  • As needed, takes documents to physician office for signature and/or delivers document to post-acute providers

Assists with coordination of information such as:

  • Discharge date, disposition, discharge instructions/orders and/or discharge summary to post-acute providers
  • Communicates pertinent patient information to post-acute providers and 3rd party payers.
  • Obtains medical records for submission to receiving facilities, 3rd party payers, and QIO as requested by Care Coordinators and Social Workers.

Maintains (keeps updated and/or well-stocked) computer and paper files including but not limited to:

  • Resource Boxes on each unit
  • Information Booklets
  • Documents in Shared Drive

Assists with data collection and statistical report preparation for:

  • Departmental Quality Improvement
  • Quality Coordinating Council
  • Utilization Review Committee

Assists with timely Departmental Communication by:

  • Taking telephone messages/checking voicemail for the Case Management department
  • Receives fax and printer communications and conveys to the appropriate person(s) in a timely.
  • EMS Convalescent
  • Taxi
  • Wheelchair transport
  • Care Transitions Clinic Appointments
  • Community Clinic Appointments

Assists with other tasks or duties as requested by the Director of Case Management and Care Transitions


Shift: Saturday and Sunday 8:00 AM - 4:30 PM


Requirements

High School Diploma or equivalent required

Valid North Carolina Driver’s license

1-2 years clinical and/or clerical experience preferred

Knowledge of medical terminology required

Knowledge of operation of personal computer and Microsoft Office Suite

Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds. Visual acuity, hearing, stooping, reaching and kneeling.
