Under the direction of the Rehabilitation Director, evaluates, plans, and administers medically prescribed physical therapy treatments to inpatients and outpatients.
Essential Job Functions:
· Performs a comprehensive physical, psychosocial, and safety assessment of assigned patients as evidenced by clinical documentation.
· Develops a Plan of Care to promote optimum function and safety as documented in the clinical record
· Coordinates patient care with other providers ensuring optimal patient care and effective communication as noted in the clinical record.
· Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.
· Follows hospital exposure control plans/blood borne and airborne pathogens.
· Maintains confidentiality of all hospital and patient information at all times. Follows HIPAA regulations and policies.
· Regular and predictable attendance is an essential job function.
· Communicate with patients, families and co-workers in person, in writing and on telephone.
Minimum Qualifications:
· BS Degree in physical therapy (PT)
· Current state PT license
· CPR certification
· Basic understanding of physical therapy services across the spectrum of inpatient and outpatient care.
Full time, 40 hours per week
