Job Description

Job Objective: A Registered Nurse is responsible for the nursing care of patients as established by the Nursing Practice Act of the Board of Registered Nursing of the State of California and as guided by policies and procedures of the Medical Center.

 

Duties and Responsibilities:

  1. Nursing Process

·         Apply nursing process in direct patient care.

·         Provides accurate assessment and documentation of physiological, psychosocial, cultural, developmental, socioeconomic, and spiritual needs of patients as evidenced by assessment within time frame appropriate for level of care

·         Provides logical sequence of action, updates nursing care plan as evidenced by notations on care plan, provides evaluation each shift of patient's progress in relation to the patient care plan, if applicable to area of care

·         Provides Information to patient/family relating to educational resources available, Directs patient/family educational process

·         Provides discharge planning beginning on admission with appropriate referrals and update as required by the patient

·         Adheres to the admission and transfer criteria

·         Adheres to all current policies and procedures in providing patient care and seek clarification if needed.

·         Evaluates and documents outcomes of medical, nursing, and ancillary team interventions

·         Incorporates time management skills in accomplishing care

  1. Nursing Care Delivery Process

·         Knowledgeable regarding current nursing practices and procedures

b.  Demonstrates knowledge of Medical Center structure and administrative resources

c.  Demonstrates ability to interpret rules and regulations

·         Acts as liaison between medical staff and patient care team

·         Assesses safety/cleanliness of patient/unit environment and reports conditions requiring change

·         Utilizes resources, equipment, and supplies efficiently and with cost awareness

·         Reports hazardous conditions to appropriate department for correction

·         Uses clean and aseptic techniques for protection of patients and personal safety

·         Prevents contamination and/or transfer of Infection by use of the Exposure Control Plan and Standard Precautions in daily work

·         Demonstrates knowledge of emergency codes

Education and Experience:

Education

·         Adheres to required educational standards

·         Maintains current licensure, certifications, education, and physical health standards Participates in mandatory and/or optional educational activities/offerings. Attend staff meetings regularly and as required by assigned unit

·         Demonstrates knowledge and competency on hospital information technologies used in the designated patient care area.

·         Successfully completes written and practical competencies annually or otherwise as assigned.

Experience

·                     Two years' acute care nursing preferred; new grad training provided as appropriate

 

Required Licensure and/or Certifications:

·         Current California Registered Nursing license

·         BCLS Certification

·         ACLS, PALS, and/or other additional Certifications required for some specialty areas

*Resuscitation credentials such as (BLS, ACLS, PALS, etc.) are required before completion of orientation period.

Job Objective:

The Rapid Response Team (RRT) is a Critical Care Registered Nurse who is responsible for responding to rapid Response Calls.

 

Duties and Responsibilities:

The RRT meets all the essential duty requirements of a Critical Care Registered Nurse.  Additionally, the RRT must exemplify expert clinical skills, effective planning, leadership abilities, and ensure appropriate delivery of patient care according to the established standards.  The RRT must work effectively with an interdisciplinary team and possess strong communication skills.  The RRT must report to the House Supervisor every shift.

  1. Rapid Response Team: Primary Focus
    1.  Respond to Code Blue calls in the main hospital and in the WIP.
    2.  Respond to Code Stroke calls in the main hospital and in the WIP.
    3.  Evaluate and facilitate, with MD, treatment plan for RRT patients.
    4. Working knowledge of proper level of care placement.
    5. Provide expert consultations for deteriorating patients in the main hospital and in the WIP.
    6. Use standardized order set to treat the patient requiring care by the Rapid Response Team.
    7. Assume primary care of patients waiting placement in higher level of care.
    8. Resource nurse for patients waiting for placement in higher level of care.
    9. Provide support for ED admits awaiting admission to CCU and PCU.
    10. Round on all inpatient patient care areas throughout the shift (main hospital and WIP).
    11. Follow up on CCU downgrades and floor downgrades.
    12. Respond to calls for change in patient’s condition.
    13. Assist in CCU as needed.
    14. Assist with CCU and PCU patients who require moderate sedation during procedures provided off the CCU and PCU units.
  2.  Critical Care Team Member: Secondary Focus
    1.  Cover patients in CCU when primary nurse is off the unit.
    2.  Patient care.
    3.  Assist with bedside procedures.
    4. Collaborate with the CN of CCU and assist as needed.
    5. Complete RRT and/or response documentation in the Electronic Medical Record.
    6. Sepsis Surveillance.
    7. Review Vigilanz Reports.
    8. Maintain Rapid Response Data Base in Midas.
    9. Act as critical care transport when need arises for emergent transport of a critical care patient that needs a higher level of care.  
  1. Nursing Care Delivery Process

    3. Demonstrates knowledge of staffing guidelines for CCU.
    4. Demonstrates knowledge of conscious sedation.

·         National Certification in Critical Care Nursing (CCRN) highly preferred

  • Ability to frequently lift, push and pull up to 25 lbs.
  • Ability to occasionally assist, as part of a team, in lifting, pushing and/or pulling patients for the transfer of care, requiring exertion of up to and over 50 lbs.
