Position Overview:

As an integral member of our healthcare team, the Registered Nurse (RN) at Seva Adult Day Care, Seva Home Care and Seva Structured Family Caregiver is responsible for providing compassionate and professional care to seniors. The RN will coordinate care, assess healthcare needs, and ensure the well-being of our Seva Seniors. 

Experience:

• Possess a minimum of 2 years of nursing experience (Required).

• Experience in an Adult Day Care setting (Home Care & Structured Family experience is a plus)

License/Certification:

•  Maintain and provide an active state license as a Registered Nurse (RN) (Required).

•  Maintain and provide an active BLS/First Aid Certification (Required).

Qualifications and Compliance:

•  Possess a minimum of 2 years of experience in working with seniors.

•  Demonstrate knowledge of Home Care Software Experience.

•  Hold a valid driver's license.

•  Maintain reliable transportation and insurance.

•  Ensure compliance with COVID-19 vaccination requirements (as needed).

Schedule and Compensation:

•  Adhere to a Full-time schedule.

•  Receive competitive wage options based on experience.

Care Coordination:

• Coordinate Initial Assessments and Supervisory Visits for seniors.

• In-home assessments & nurse consulting for health coaches.

•  Coordinate care with the interdisciplinary team, clients/families, and referring agencies in the Day Care setting.

•  Utilize the nursing process to assess, plan, and intervene to meet the client's healthcare needs effectively.

•  Assume responsibility for the coordination of care while in our Day Care, ensuring a holistic and individualized approach.

•  Offer emotional support to seniors, creating a warm and caring environment. 

•  Provide medication (as needed) as prescribed by a physician, administer injections, and monitor vital signs.

•  Assess patients' reactions to medications and their mental health, offering emotional support as needed.

•  Observe and report any signs or symptoms indicative of changes in the client's condition and family situation, documenting observations in the clinical record.

Documentation and Record-Keeping:

• Document client activities and enter information into computer systems accurately.

• Maintain up-to-date and comprehensive medical records.

• Caregiver Timesheet Approvals

•  Exhibit exceptional staff training and customer service skills.

•  Demonstrate a compassionate and friendly disposition in interactions with seniors and team members.
