Glenmeadow at Home : We are seeking skilled professionals who are prepared and eager to provide exceptional Care. Glenmeadow at Home offers personalized care to those we serve throughout the Greater Springfield community.

Essential Duties, Attitudes and Responsibilities include the following:

• Assist in carrying out the care plan for each client.
• Assure the safety, physical and mental well-being of each client.
• Provide companionship to the extent needed and possible.
• Supports activities of daily living (ADL) as a way to maximize client’s independence and well-being.        
• Incorporates an understanding of the aging process and individual preferences into ADL support.
• Transfer, transport or assist residents to ambulate as needed.
• Perform personal services for residents such as errands, greeting visitors, and answering the telephone.
• Perform light housekeeping or cooking duties as written in the service plan.  Light housekeeping may include dusting, vacuuming, changing linens and laundry.
• Facilitate healthy nutrition by assisting the individual with meal planning, food preparation and serving, and food shopping and handling, in accordance with the individual’s preferences and plans.  Ensures the individual is well nourished.  Understands the importance of good nutrition and hydration.
• Assist with errands and escorts as needed.
• Assist with pet care as needed.
