Implements effective strategies for linking uninsured and underinsured clients to primary care, increases access to appropriate medical care and community-based services, assists individuals in improving health status, and promotes behavior change in using Morton health care services.
Requirements:
- High School Diploma or GED required.
- · Healthcare, social work, or public health background preferred but not required.
- · Understanding of the healthcare delivery process is required.
- · Excellent interpersonal communication skills are required.
- · Broad knowledge of community resources and services is required.
- · Bilingual (English/Spanish) profiency required.
- Excellent computer skills are required
- Required to maintain a high level of confidentiality.
- · Demonstrated ability to work with community partners, patients, and staff professionally and cooperatively.
- · Ability to work in a fast-paced environment.
- · Strong attention to detail.
- Ability to work independently as well as part of a team.

- Must have and maintain a valid driver’s license.
