Afternoon Transportation & Individual Support (Dayton → Keyport)

Description:
ASG Home Care NJ LLC is seeking a reliable Direct Support Professional (DSP) to support a 66-year-old female individual with transportation and community-based support needs. The individual lives in Keyport and attends a day program in Dayton (Dawn 2 Dusk). This role will primarily involve afternoon pick-up from program and safe transport home, with the potential for additional afternoon or weekend hours depending on fit and availability.

This case will be billed under Individual Supports (not transportation), as support is provided during and after transport.

Schedule & Hours as the Direct Support Professional:
• Approx. 10–15 hours per week (minimum)
• Primarily weekday afternoons (program pick-up)
• Additional afternoon/weekend hours may be available

Location:
• Pick-up: Dayton, NJ (day program)
• Drop-off: Keyport, NJ
• Distance: approx. 20–30 miles

Responsibilities as a Direct Support Professional:
• Provide safe, reliable transportation from program to home
• Provide supervision and support during transport and after drop-off as needed
• Assist with light community or in-home support when scheduled
• Maintain professional communication with family and agency staff
• Complete required documentation and EVV requirements

About the Individual:
• 66-year-old female (Tier D)
• Lives with aging parents
• Very sweet; responds best to staff who are patient, kind, and gentle

Requirements as a Direct Support Professional:
• Valid driver’s license and reliable vehicle
• Clean driving record
• DSP experience preferred (IDD experience strongly preferred)
• Must meet all NJ DDD onboarding requirements (fingerprinting, trainings, etc.)
