  • At least one year of experience as a Supports Planner, with a full understanding of different Medicaid waiver processes is required.
  • Bachelor�s degree in human service focus (psychology, social work, public health, etc.) is strongly preferred.
  • Ability to quickly process client files to provide quality services and continuity of care.

What You Will Do:

The Flex Supports Planner provides coordination of services to individuals in the Community First Programs (CFP) Division, covering a variety of Supports Planning responsibilities across all regions to support program needs. The Flex Supports Planner supports the mission of The Coordinating Center (The Center) by providing person-centered coordination services and adhering to the parameters of The Center. Flex Supports Planners are responsible for complying with specific contract requirements, integrating technology to maintain database integrity, report writing and maintenance of documentation. The Flex Supports Planners report directly to their assigned Region-Based Team Manager and work interdependently with the Community First Programs Team to maximize quality outcomes for individuals.

As a Flex Supports Planner, you will:

  • Utilize assessments and information from various sources regarding medical, psychological, and social factors to evaluate and advocate for client needs.
  • Develop an individualized person-centered plan of service for each client, recommending supports and services.
  • Use critical thinking skills to troubleshoot any conflicts or barriers within the support team including service issues and client crises.
  • Perform quarterly on-site visits to homes, nursing facilities, and community locations and monthly telephonic care coordination.
  • Locate appropriate resources for clients and work collaboratively with community agencies and providers to achieve desired client outcomes.
  • Assist individuals with developing life goals and monitors progress to ensure health and safety in the community.?
  • Obtains and monitor estimates from various service providers including medical equipment and supplies.
  • Adhere to contractual billing guidelines when documenting all activities and tasks, including entering case notes and other documentation in multiple electronic database systems.
  • Assist in client transitions from medical facilities to a community setting.
  • Prioritize, develop, and manage your own schedule to ensure timely completion of work.
  • Makes recommendations to improve programs, procedures, and effectiveness of service delivery to target population.
  • Manage multiple priorities and tasks with flexibility to enhance service coordination services.
  • Establish and maintain working relationships with state and county agencies and law enforcement agencies to facilitate working effectively on problems of the population served.
  • Assist with mentoring and training newly hired Supports Planners as needed.
  • Attend regular staff meetings, in-services, and other training to enhance skills and professional development.
  • Understand that caseload may fluctuate based on coverage needs throughout the program, requiring completion of file transfers frequently.
