Position Overview

The MDS Coordinator is responsible for the timely and accurate completion of the RAI/MDS process from admission through discharge. This role works closely with the interdisciplinary team to ensure regulatory compliance, accurate reimbursement, and high-quality resident care.


Key Responsibilities

  • Coordinate and manage the RAI/MDS process in compliance with federal and state regulations

  • Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews

  • Ensure accurate MDS coding supported by appropriate clinical documentation

  • Monitor resident status and identify significant changes impacting care plans and assessments

  • Maintain accurate and timely data entry in PointClickCare and other MDS systems

  • Submit MDS documentation to the state and manage validation error follow-up

  • Participate in care plan meetings, quality assurance activities, and survey readiness

  • Track discharges, re-entries, Medicaid, and PPS requirements

  • Report audit findings and trends to the Quality Assurance Committee


Qualifications

  • Graduate of an approved LPN or RN program; active state license required

  • 2+ years of skilled nursing experience preferred

  • Working knowledge of MDS 3.0, RAI process, OBRA regulations, and care planning

  • Knowledge of Medicare PPS, Case-Mix, and Medicaid reimbursement

  • Experience with PointClickCare preferred

  • Strong organizational, documentation, and communication skills
