Description
As a Program Coordinator, you will primarily supervise team members and Client Service Coordinators (CSCs) who work directly with individuals with intellectual disabilities, ensuring they are supported in achieving fulfilling and independent lives. Program Coordinators may need to provide direct support to individuals with disabilities; your main focus is providing leadership, structure, and guidance to CSCs and direct support professionals. You will ensure services are delivered with quality, respect, and consistency. Your role is to encourage independence, promote choice, and create an environment where individuals thrive within their communities.
Responsibilities:
Under the guidance and direction of the Director, the Program Coordinator, QIDP will:
- Implement the client's individual plans as authorized and agreed upon by interdisciplinary team members.
- Ensure that eligibility for services provided by ASI is established and maintained.
- Oversee and quality-assure the collection and storage of client information related to eligibility, habilitation planning, and program development and implementation.
- Develop service plans and transition plans and oversee the implementation and monitoring of services.
- Develop, write, monitor, and revise habilitation plans based on the needs and wants of the individual in service.
- Develop and oversee the development of progress reports related to the provision of services per company standards.
- Provide training to employees on the implementation of the habilitation plan and provide ongoing monitoring of outcomes from the plan.
- Provide training and support so persons served may live as independently as possible, including response to dangerous or emergency situations.
Program Coordinators coach team members and persons served in the development of daily living skills training, including:
- Establishing positive relationships with community members.
- Identifying activities of personal interest.
- Facilitating client participation in community activities.
- Identifying specific training activities to participate on an on-going basis.
- Mobility training or assistance aimed at enhancing movement, accessing and using public transportation, mastering the use of adaptive aids and equipment.
- Document services provided to demonstrate achievement towards individual goals and outcomes.
- Participate in the planning, coordination, and presentation of employee training, including New Employee Certification and Continuing Education.
- Other duties may be assigned by the supervisor.
- Hire, train, schedule, and supervise team members.
- Manage program budgets and monitor financial performance.
- Promote continuous improvement in service delivery and staff performance.
Requirements
Specific Requirements:
- Bachelor’s degree (required).
- Degree in human services, psychology, social work, or related field (preferred).
- 24/7 on-call
- Minimum 3 years of experience in human services, with at least 1 year in a supervisory role.
- Strong organizational and leadership skills.
- Ability to manage budgets and meet program goals.
- Proficiency with Microsoft Office Suite and related software.
- Excellent written and verbal communication skills.
- Commitment to supporting individuals with intellectual and developmental disabilities.
