Job Summary:

The Children’s CCBHC Care Coordinator plays a vital role within the Certified Community Behavioral Health Clinic (CCBHC) model. This position is responsible for ensuring that children, youth, and their families receive seamless, integrated, and high-quality care. You will act as the "bridge" between mental health services, primary care, schools, and social services, focusing on a family-driven, youth-guided approach to recovery and wellness.

Duties/Responsibilities:

  • Care Coordination & Advocacy: Facilitate communication and collaboration between the family, the CCBHC clinical team, primary care providers, and external partners (schools, juvenile justice, child welfare).
  • Assessment & Planning: Assist in the development of comprehensive, person-centered, and family-driven treatment plans. Ensure all services align with the child’s specific developmental and emotional needs.
  • Follow up and Monitoring-regular check ins to assess and ensure client needs are being met. 
  • Resource Navigation: Connect families with community-based supports, including housing assistance, food security, specialized educational services (IEPs/504 plans), and extracurricular programs.
  • Crisis Support: Collaborate with the crisis team to ensure children experiencing mental health emergencies have follow-up care and a robust safety plan in place within 24 hours of an episode.
  • Transition Management: Support the family during "warm hand-offs" between different levels of care (e.g., transitioning from inpatient psychiatric care back to the home/school environment).
  • Documentation: Maintain accurate and timely electronic health records (EHR) that meet CCBHC quality standards and state/federal compliance regulations.

  • Recognizes and understands mental health symptoms and diagnoses.
  • Skilled in using essential office hardware and software, such as scanners, shredders, phone systems, and email platforms.
  • Understands the confidential nature of organizational information and maintains confidence.

Education and Experience:

  • We welcome candidates from all educational backgrounds. While no degree is required to apply for this position, a 2-year or 4-year degree in a related field will qualify the candidate for a higher starting hourly wage. We are committed to recognizing both formal education and practical experience in our compensation reviews. 

Physical Requirements:

Employee must be able to:

  • Hear and speak in order to use telephones and communicate with people inside and outside the office.
  • Physical ability to move around the office area and in the community throughout each workday.
  • Visual ability to review detailed reports and information, and to use the computer for keyboarding.
  • Bend at the waist or crouch for filing charts in the filing cabinets.
  • Physical ability to Exert up to 20 pounds or up to 10 pounds of force frequently.

Work Environment:

This position operates on a hybrid schedule, requiring two days in office and offering 3 days of remote work flexibility. Candidate will work within an integrated, multidisciplinary team environment, requiring frequent coordination with medical, behavioral health, and social service professionals to ensure holistic client care.
