Service Areas: Greater Nashua and surrounding communities

What You’ll Do

Provide hospice and palliative care visits and consultations

Deliver expert pain and symptom management

Serve as a resource for community clinicians

Support development and maintenance of clinical standards

Participate in on‑call rotation

Educate and support hospice staff using evidence‑based practice

Build strong relationships with community providers

Complete timely evaluations, documentation, and coding

Maintain ongoing professional education


Minimum Requirements

Master’s or postgraduate NP degree

Active RN and APRN licenses (NH)

Unrestricted DEA and Controlled Substance License

Eligible for hospital privileges

BLS certification

Strong communication and teaching skills

Satisfactory background and driving records; active driver’s license and auto insurance


Preferred qualifications

Existing local hospital privileges

Advanced Hospice & Palliative Care Certification

Expertise in pain/symptom management and palliative pharmacology

Experience supporting psychosocial needs of terminally ill patients and families
