Job Description

CNA - Home Health & Hospice

Service Area with Openings: Southwest Houston

Responsibilities:

  • Provide exceptional care in coordination with the assigned nursing care team.
  • Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
  • Help with patient meals, feeding, and basic activities of daily living.
  • Deliver weekly reports to the case manager and review any important changes in patient conditions.
  • Educate patients and family members.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate effectively with healthcare providers and other care team members.

Qualifications:

  • CNA license in the state of Texas (required).
  • One (1) year hospice experience.
  • Able to lift up to fifty (50) pounds.
  • Able to work occasional weekends, holidays, evenings.
  • Strong communication and people skills.
  • Experience with HCHB EMR (preferred but not required)
