We are seeking an experienced RN or LPN to serve as our Full-Time MDS Coordinator, joining the Nursing Administration Team to ensure compliance, optimize reimbursement, and promote person-centered care.

Key Responsibilities
- Lead the RAI/MDS process including person-centered care planning, PDPM reimbursement, Medicare certification, scheduling, month-end triple check, and appeals
- Maintain PDPM/Medicare and Medicaid assessment schedules and distribute to therapies, dietary, social service, activities, and nursing teams
- Provide ongoing education and training on MDS and care planning
- Oversee the RAI process, including MDS assessments, triggers, CAAS, and care planning
- Maintain Medicare certification/recertification binder to ensure compliance with federal Part A & B services
- Stay current on long-term care regulations, best practices, and reimbursement requirements
- Support the facility in audit and educational tools, including template development
- Utilize Microsoft Office (PowerPoint, Word, Excel, Outlook) for reporting, tracking, and training
Requirements
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid New Hampshire license
- RAC-CT certification preferred
- Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning
- Must have strong clinical and communication skills
- Extensive knowledge of the RAI process and reimbursement requirements for both federal and state
- Must be highly organized with the ability to multi-task throughout the day
- Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred
- Experience in developing templates to utilize in facility for audit/educational use
