We are seeking a detail-oriented and organized Licensed Practical Nurse (LPN) to join our team as a Quality Assurance Assistant in a home health setting. This role supports compliance, documentation accuracy, and quality improvement initiatives to ensure exceptional patient care and adherence to regulatory standards.

Key Responsibilities

• Review clinical documentation for accuracy, completeness, and compliance with state and federal home health regulations
• Assist with quality assurance audits and performance improvement activities
• Track and report quality metrics, trends, and outcomes
• Collaborate with clinical staff to ensure proper documentation and timely corrections
• Support survey readiness and accreditation processes
• Maintain patient confidentiality and adhere to HIPAA guidelines
• Assist in policy and procedure updates as needed

Qualifications

• Current, active LPN license in Missouri
• Minimum of 2 years of experience in home health, hospice, or related field preferred
• Knowledge of home health regulations (e.g., Medicare Conditions of Participation)
• Strong attention to detail and organizational skills
• Proficiency in electronic medical records (EMR) systems
• Excellent communication and teamwork skills

Preferred Skills

• Experience with OASIS documentation and review
• Familiarity with quality improvement processes
• Ability to analyze data and identify trends
