LPN/RN Nurse Case Manager - Genesee Senior Living


Job Schedule: Monday- Friday 8am-4:30pm
*This position will require some on call duties, unit coverage over weekends and/or holidays.

LPN/RN Nurse Case Manager Responsibilities:

  • Assess patients' needs and develop individualized care plans to promote optimal health and well-being

  • Coordinate with healthcare providers, families, and caregivers to ensure seamless transitions and continuity of care

  • Monitor patients' progress, make adjustments to care plans as needed, and communicate effectively with all stakeholders

  • Provide education and support to patients, families, and caregivers to promote independence and healthy habits

  • Collaborate with interdisciplinary teams to develop and implement effective care strategies and protocols

  • Document all patient interactions, assessments, and care plans in accordance with facility policies and regulations

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

LPN/RN Nurse Case Manager Requirements:

  • Current LPN/RN license in the state of New York

  • Prior experience in a healthcare setting, preferably in a case management role

  • Strong assessment, communication, and problem-solving skills

  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks

  • Strong attention to detail and organizational skills
