Job Summary
Treat actual or potential health problems using the nursing diagnosis and case management. Teach, counsel and provide supportive care to the patient and their significant others
Essential Functions
Performs a thorough and timely assessment of patient’s condition including physical, psychological, social and rehabilitative aspects.
Identifies problems/nursing needs including discharge needs
Intervenes appropriately to address identified problems and needs. Plans care according to standards of care. Manages time efficiently/sets priorities appropriately
Evaluates effectiveness of care and revises plan if necessary
Provides education consistent with the patient/family learning abilities and preferences; evaluates outcomes
Effectively communicates with physician’s, co-workers, patients, families
Reports pertinent observations regarding patients to physicians and other health care team members
Shift report is given on time, accurately and concisely
Documentation on the patient’s medical record is accurate, complete and concise
Maintains National Patient Safety Goals
Verifies, coordinates and carries out physicians orders accurately
Coordinates interdepartmental transfers smoothly and efficiently
Coordinates interdepartmental activities (i.e., lab, radiology, etc.) to ensure timely efficient care
Communicates and coordinates with Case Manager to ensure timely discharge, patient satisfaction and quality care along the continuum of care
Practices Universal Standard Precautions
Demonstrates knowledge of practices department safety procedures, including safe handling of equipment and hazardous substances

Minimum Education/Certifications
Graduate of an accredited program in nursing.
Current Registered Nurse license to practice in the state of NJ.
Certifications required: BLS (American Heart Association)
Minimum Work Experience
1 year or more experience in Behavioral Health preferred.
Position Type/Expected Hours of Work: Must be willing to work holidays, weekends and provide additional coverage as needed
