CFC Intake Coordinator



Program: Child and Family Connections, Early Intervention



Location: Arlington Heights, IL



Hours: Full time, Monday - Friday, 7:30am-3:30pm (40 hours per week)



Pay: $21.00-$23.00/hr



Clearbrook is a non-profit, social services agency that specializes in providing innovative opportunities and services to individuals with intellectual and developmental disabilities.



The Intake Coordinator is responsible for facilitating telephone, fax and mail activity. Maintain program forms and public awareness materials. Complete initial intake phase of all referrals. Coordinate evaluation schedules for team evaluators. Create new client files.



Essential Functions:



Secretarial: 



 



1. Receive and direct incoming phone calls in a cordial, professional manner.



2. Receive and distribute mail to appropriate staff.



3. Maintain an adequate supply of forms and informational materials.



4. File administrative and client files.



5. Support database activity.



6. Type material as requested.



7. Coordinate some provider evaluation schedules.



Intake: 



 



1. Receive all intake calls, gather pertinent information from the caller.



2. Follow up on all referrals and complete the initial intake information for children referred by outside referral sources.



3. Mail/fax medical authorization forms to primary care physician.



4. Mail intake packet containing consent forms to family/guardian.



5. Enter all intake information into Cornerstone.



6. Reopen, assign, and enter information into Cornerstone for children re-referred.



7. Create new client files.
