Position Summary



The role of the Registered Nurse - Home Health Visit reports to the Nurse Supervisor. This position is responsible making home visits, providing direct patient care, and completing related paperwork.



 



Essential Position Responsibilities




  • Assess and chart observations of the patient’s condition at each visit

  • Complete evaluation tasks, including reviewing medication and vital signs

  • Administer physician-prescribed medication

  • Through proper wound care procedures, document and assess heading progress

  • Complete written reports on patients in a timely manner

  • Provide thorough education for patients and families on proper home health care procedures and strategies

  • Partner and coordinate care with members of the patient’s care team

  • Recommend resources that may improve the quality of life for the patient and assist patient in maintaining their independence

  • Update care plan and orders accordingly



Qualifications




  • Education: Must be a graduate of an accredited nursing program

  • Licensure and Certifications: RN license and CPR, required. Valid driver’s license and auto liability insurance, required.

  • Experience: Minimum of one year of clinical nursing experience, required.



Work Environment: This position has exposure to communicable disease, body fluids, and radiation. This position will spend 60% of time standing and/or walking short distances while pushing, pulling, lifting, and/or carrying up to 50 lbs. This position will spend 40% of time sitting while performing work in a standard office environment. Frequent local travel required.
