Position Title: Licensed Practical Nurse (Residential - LPN)

Department: Residential (RES)

Reports to: Residential LPN Supervisor

 

About the Program:

Residential (RES): also known as the Diermeier Therapeutic Youth Home in North Lawndale. The facility can serve 70 youth, most of whom come through DCFS, group homes, foster care and psychiatric hospitals. The around-the-clock facility is staffed by more than 150 employees who provide services to decrease problematic behaviors. The program provides life skills, tutoring, therapy, educational opportunities, programmatic activities, sporting and recreational events, camping, part-time employment opportunities, and arts and crafts, among other offerings. The goal at discharge is to have successfully engaged youth so they can transition to foster care or transitional living services. The accredited program is considered high end because of services and supports provided to youth with the most difficult behaviors. 

 

Position Objective and Summary:

In this full time position, the LPN will be responsible for the administration of psychotropic medication to clients as well completing documentation related to medication dispersal. The LPN may also assist the medical department with assessing illness in clients and completing post physical restraint documentation. 

 

This is a Full-time position with 2 available rotating weekend shifts. Please indicate on your online application which shift you are applying for.

Shift A) Monday-Sunday 6:00 am to 2:00 pm

Shift B) Monday-Sunday 2:30 pm to 10:00 pm

 

Essential Duties and Responsibilities:

  • Administers psychotropic medication to all clients as needed. 
  • Ensures that all administered medication is swallowed by the client.
  • Assess and document post restraint injury or illness.
  • Records administration of medication(s) in the client’s medical record.
  • Advises clients of dosage, name of medication, reason for prescription and possible side effects.
  • Ensure medication cart is orderly and stocked with all client’s current medication.
  • Check all medication for expiration date and dispose of properly when outdated.
  • Assess all client’s health need with consultation of the program RN.
  • Completes appropriate Medicaid documentation to support the administration of psychotropic medication.
  • Provide education to youth and staff regarding individual health issues. 
  • Checks to ensure that clients have sufficient medication(s) on the units.
  • Assesses clients, in consultation with the program RN.
  • Assesses clients for any sign of injury after any physical intervention, as needed.
  • Participates in supervisory sessions and other related program meetings.
  • Maintains current knowledge through participation in training sessions and other related program meetings, on a regular basis.
  • Performs other related tasks as required or assigned.

 

Education /Job Experience/Certification:

  • State of Illinois LPN registration.
  • Two (2) years experience in a medical or psychiatric hospital setting or other inpatient type of treatment program/center

 

Special Knowledge and Qualifications:

  • Proven ability to work effectively with children and adolescents.
  • Computer skills preferred.
