 Position Overview

The Social Work Case Manager  works closely with the Social Services Director and interdisciplinary team to ensure residents receive comprehensive, person-centered support. This role focuses on care coordination, documentation, resident engagement, and family communication.

Key Responsibilities

• Assist with resident assessments and care plan development

• Support case management documentation and compliance

• Coordinate appointments and community resources

• Provide emotional support and advocacy for residents

• Communicate with families regarding care updates and services

• Assist with discharge planning and transitions

• Maintain accurate records in accordance with state regulations

• Participate in interdisciplinary team meetings
