Position: Physical Therapist
Job Summary
Flexible hours to be consistent with providing support to individuals with developmental disabilities in their natural environments: home, day program, and community. To work as an interdisciplinary team member to implement support plans that facilitate clients in reaching their full potential. To promote professional development in the ICF/IID department. To provide critical competency based teaching to direct care management and staff to facilitate continuity of care.
Primary Duties and Responsibilities
• Function as a member of the comprehensive assessment process for clients supported in the ICF/IID department, providing data collection, analysis, and functional recommendations for at risk individuals with comprehensive needs. Maintain appropriate documentation of follow through on recommended actions.
• Perform direct therapy for individuals.
• Based upon assessment and determination of need, develop a treatment plan for each individual, which describe the purpose and proper procedure relating to all positioning, mobility, and orthotic devices.
• Implement treatment plans by providing competency based training to residential and day program staff.
• Required participation in all client Interdisciplinary Team (IDT) meetings, to include: Planning and ISP meeting documentation required to complete support plan, presentation of physical therapy assessment, development of appropriate goals as part of an interdisciplinary team.
• Coordinate with each individual’s IDT to procure and maintain in good repair all positioning, mobility, and orthotic devices which are appropriate for those individuals.
• Perform and document quarterly preventative maintenance checks on all equipment. Coordinate any needed repairs or replacement of equipment.
• Attend with individuals, as appropriate, doctor appointments, clinic visits, and meetings with equipment vendors.
• Provide Physical Therapy Supervision for Physical Therapist Assistants.
• Provide 24/7 on-call service for all PT equipment and other PT emergencies for ICF individuals.
• Assist with Orientation instruction of Challenges in Physical Management.
• Maintain written, electronic and verbal confidentiality, in compliance with HIPAA guidelines, for individuals served.
• Participate in continuing education to further professional skills related to physical therapy practice in the field of developmental disabilities, and for licensure renewal.
