About the Role

The Heart Failure Nurse Navigator plays a vital role in the patient journey, supporting individuals diagnosed with heart failure from hospital discharge through outpatient care. In this multifaceted role, you’ll act as a case manager, educator, and advocate—working closely with physicians, floor nurses, and support staff to ensure continuity of care and prevent hospital readmissions.

You’ll split your time between two primary campuses: DMOB (main office) and DHEC (hospital campus), actively engaging with both clinic and inpatient teams.

Day-to-Day Responsibilities

Patient Navigation & Advocacy

  • Coordinate follow-up care post-hospitalization
  • Educate patients and families on heart failure, medications, lifestyle, and care plan
  • Perform CardioMEMS monitoring and interpret data
  • Navigate barriers to care including insurance or scheduling issues
  • Establish long-term support relationships with patients

Hospital-Based Coordination

  • Round on floors to identify heart failure patients and collaborate on discharge plans
  • Be a visible presence on the floor and in PPR meetings
  • Collaborate with inpatient teams to ensure timely GDMT (guideline-directed medical therapy) initiation
  • Provide staff education, in-services, and training on heart failure protocols

Program Development & Quality Improvement

  • Assist with development and refinement of HF education packets
  • Educate new RN residents and cardiology fellows on heart failure protocols
  • Monitor outcomes and track patient success to inform quality improvement

Team & Clinic Support

  • Support outpatient clinic operations during staff absences
  • Make HF education packets for both inpatient and clinic use
  • Collaborate with Billing, HUCs, and leadership on follow-up processes


What You’ll Need to Succeed

Required Qualifications

  • Current Washington State RN license required
    • Must obtain an MSL and Oregon license within 90 days of hire. You will be reimbursed for this expense as a condition of your employment.
  • Bachelor’s degree in Nursing (BSN) preferred; associate degree considered with additional experience
  • 3–5 years of clinical experience (experience with cardiac or heart failure patients a plus)

Preferred Experience (please apply even if you don't have this experience)

  • Master’s degree in Nursing (a standout feature, but not required)
  • Experience developing healthcare programs
  • Knowledge of GDMT and heart failure case management
  • Strong interpersonal, communication, and critical thinking skills
  • Passion for patient-centered care and health education

Work Environment

  • Location: DMOB with frequent trips to DHEC
  • Schedule: Monday–Friday, 8:00 AM – 4:00 PM (Exempt role)
  • Environment: In-office, clinical setting with regular walking between hospital floors and clinic locations
  • Travel: Local, between partnered locations only
