BHcare is seeking a Community Support Specialist to join our team in our Ansonia Adult Outpatient Clinic! This role is all about helping clients build independence and achieve their goals through skill-building, motivational support, and connection to community resources. This is part-time position, 20 hours per week Monday-Friday from 9am-1pm. 



What You’ll Do:

• Provide individual and group sessions that promote life skills, recovery, education, employment readiness, and community involvement.

• Conduct functional assessments and develop person-centered recovery plans with clear, achievable goals.

• Deliver stage-wise interventions using the psychiatric rehabilitation skill-building model.

• Support young adults with daily living skills, health and wellness education, and self-advocacy.

• Provide crisis intervention, outreach, and ongoing support in both individual and group settings.

• Help clients connect with medical, psychiatric, and community appointments.

• Educate and collaborate with families, significant others, and community members to support recovery goals.

• Provide limited transportation while helping clients build their own transportation independence.

• Maintain accurate clinical records, progress notes, and recovery plans in compliance with agency standards.



What We’re Looking For:

• A compassionate, reliable professional who wants to empower young adults toward independence.

• Strong communication, problem-solving, and teamwork skills.

• Experience in human services, behavioral health, or rehabilitation strongly preferred.

• Familiarity with recovery-oriented approaches and motivational interviewing is a plus.

• Valid driver’s license and using personal vehicle required.
