Requirements

Applicants must hold an active Indiana license in their respective discipline (Physical Therapist) or be eligible to obtain Indiana licensure. Eligibility includes successful completion of an accredited educational program appropriate to the discipline, passing the required national and/or state licensure examination(s), and meeting all [state] licensure requirements.
