Responsibilities:

• Deliver one-on-one skilled nursing care in home settings (wound care, catheter care, medication administration, symptom management)
• Perform assessments, evaluations, and skilled observation
• Educate patients and caregivers
• Collaborate with healthcare team for cohesive care

Requirements:

• Current WA LPN license
• BLS certification (AHA)
• Valid driver’s license and insured vehicle
• Home Health or Hospice experience preferred (not required)
