Wellness Nurse Manager / Director
Department: Home and Community Based Services / Private Duty
Reports To: HCBS Executive Director
Position Summary
St. Camillus is seeking a compassionate and experienced Wellness Nurse Manager/Director to support the Wellness Program and Private Duty Services for our Independent Living residents. This role oversees day‑to‑day wellness operations, provides clinical support through a Wellness Clinic, delivers resident education, and responds to health‑related concerns and emergencies.
The ideal candidate is a collaborative nursing professional who is comfortable working independently, supporting change, and partnering closely with Private Duty and campus leadership to promote resident wellness and safety.
Key Responsibilities
- Staff the Independent Living Wellness Center during designated hours for resident consultations, blood pressure checks, and health education
- Plan, support, and implement wellness and educational programs for residents
- Participate in pre‑admission evaluations to support smooth resident transitions
- Develop and update plans of care for Private Duty clients
- Conduct supervisory visits for Private Duty caregivers and support caregiver onboarding and training
- Collaborate closely with the Private Duty Manager to meet resident needs
- Manage infection control efforts, including outbreak management, in coordination with campus Infection Control and Occupational Health
- Respond calmly and appropriately to emergency situations and support emergency preparedness planning
- Coordinate resident and family meetings related to changes in level of independence; document and communicate concerns as needed
- Support discharge planning and assist residents transitioning to other levels of care on campus
- Maintain confidentiality and adhere to all St. Camillus policies, procedures, and code of conduct
- Perform other duties as assigned
Qualifications
- Active Wisconsin Nursing License (LPN preferred)
- Minimum 2 years of nursing experience, preferably in a senior living or community‑based setting
- Strong interpersonal, communication, and organizational skills
- Ability to work independently and collaboratively in a team environment
- Proficiency with Microsoft Office (Word, Excel, Outlook) and electronic medical records
- Strong problem‑solving, conflict management, and decision‑making skills
- Ability to respond effectively in emergency situations
- Ability to safely perform essential job functions with or without reasonable accommodation
